Customer Customization Form

A proof of submitted information in the chosen font and card layout will be emailed to the Customer's email as listed.
Please be sure your email address is correct to expedite this order. For any questions, please email Kirby Creations.

Each set of 5 invitations/cards includes 5 coordinating envelopes in either white or ivory. Magnets, Adhesive Stickers
and printing of return and mailing addresses can be included but additional charge may apply.

After this form is received and processed, a confirmation email will be sent within 48 hours along with a final invoice
that will include Shipping & Handling. Standard S&H is $6.00 per set of 5 cards and $1.00 for every set above 36.
Insurance is optional up to 35 cards. Kirby Creations is not responsible for lost, damaged, late or misdirected mail.
Orders of 36 or more cards will include insurance.
